By Our Special Correspondent

NEW DELHI, FEB. 4. The Commissioner of Railway Safety, Eastern Circle, has blamed both equipment and staff failures for the collision of banking pilot engine with the Varanasi-Asansol passenger train on January 17.

Four persons were killed and 15 injured, five of them seriously in the accident which occurred at the Down Home signal of Yadugram Block Hut on the Gaya-Gomoh section of the Dhanbad Division of the East Central Railway.

The Commissioner of Railway Safety, Mahesh Chand, who conducted the inquiry, held that the accident occurred due to "failure of equipment-locomotive and failure of railway staff."
