New forms for passport applications

By Our Special Correspondent

KOZHIKODE, JULY 31 . Only applications for passports submitted in the new forms would be accepted from October 1, the Kozhikode Passport Officer, P. Balachandran, has said.

The Ministry of External Affairs has introduced new passport application forms with a view to making them applicant-friendly.

Although the new forms have already come into effect, the existing forms would continue to be accepted till September 30 this year.

From now onwards there would be two types of forms. Applications for new/reissue/duplicate passports are to be made in Form No 1. Form No 2 is for applications for miscellaneous passport-related services. Form A costs Rs.10 and Form 2 Rs.5.

Shorter application

The new passport application is shorter with only five pages. The applicant has to fill only the first four pages.

The form should be filled first and then two clear photocopies of the form made, thereby making three sets; then photograph and signature should be affixed in original in boxes provided in all the three sets.

New stipulations

The stipulation for filing affidavits in some cases has been withdrawn and some affidavits have been amalgamated.

Other important points that the applicant should bear in mind are - recent unsigned colour photographs showing full face against dark background should be pasted in the given boxes; one copy each of the required documents should be attached with the original application as well as the two photocopies.

If applicant has stayed at more than one address during the last one year and two additional photocopies of the form for each additional place of stay should be furnished.
