The team conducted surprise inspections at Shubha Diagnostic Centre and Raghava Diagnostic and Research Centre, both in 4th Block, Jayanagar and found that they were not maintaining Form F in the way mentioned in the PCPNDT Act and details of antenatal check-ups were not recorded. The pregnant woman's family history, including the number of children she has, is recorded in Form F."They had not maintained records of antenatal check-ups done on expectant mothers and the signature of the patients and the doctor, were not present on the forms, even though they were mandatory under the Act," a member of the commission said.
