Form-5 for all employers

The Employees State Insurance Corporation’s sub-regional office has said that as per a notification issued on March 11 following an amendment to the rules, every employer should append in the revised format (form-5) a self-issued certificate in Return of Contribution’ which needs to be submitted every six months.

Those who fail to append this will be treated as defaulters and action initiated against them. For details, contact sub-regional office, ESIC, Gandhinagar.
