Why rickety vehicles?

Western countries have sleek vehicles to transport victims of accident and terrorist violence to hospitals. Pakistan too is fast catching up with this trend of providing emergency transport. But it is pathetic to see blast victims in our country being taken to hospitals in rickety old auto-rickshaws and vehicles.
